Kerang District Health provides health and wellbeing services to residents of the Shire of Gannawarra.
The staff practice a wellness and reablement approach, with a focus on frail older people and assisting them to maximise their independence and enable them to remain living safely in their own home.
All services are structure with a focus on client strengths and personal goals to support independence.

District Nursing Service
The District Nursing Service provide nursing support to Gannawarra Shire residents.
The office location is in the Allied Health Building at Kerang District Health.
Services available include:
- Health and Wellbeing Monitoring
- Blood Pressure Monitoring
- Blood Glucose Monitoring
- Wound Management
- Foot Care
- Continence Management
- Consultation and referral to other service providers as required
Palliative Care
Palliative Care is care that helps people live their life as fully and as comfortably as possible when experiencing a life-limiting or terminal illness.
The District Nurses are experienced in the provision of end-of-life care and palliative care. Their focus is on providing person and family-centred care for any individual with a life limiting illness.
The primary goal is to optimise quality of life.
The staff work closely with General Practitioners, Specialists and the Regional Palliative Care team
Home Visits for all of the above services can be arranged as required.

Social Support – Centre Based
Social Support focuses on a wellness and reablement model of care, fostering the independence and ability of all participants.
The Social Support program works with the client/ family/ carer or support services in tailoring activities and programs that support your
interests, such as:
- Helping you get out and about in the community
- Creating social opportunities
- Enjoying a meal together
- Increasing your personal fitness and strength
- Including the cultural needs of all participants
- Promoting health and wellbeing
The Social Support program runs weekdays
Transport options are available.
Social Support – Exercise
The program is designed to combine low intensity and low resistance exercises with education in a fun and social environment.
All participants are individually assessed by our staff and engaged in the creation of an exercise program that suits their individual needs.
The classes are open to everyone, though people specifically encouraged to participate are:
- Men and Women over 60 years
- Those who are currently not involved in regular physical activity
- Those people who have had falls or have a fear of falling
- People recovering from illness or surgery (if deemed safe by your GP)
The program runs Tuesday, Wednesday and Thursdays from the WD Thomas Day Centre Gymnasium.
What Will Regular Exercise Achieve?

- Increased confidence with mobility
- Increased bone density
- Reduces the incidents of falls
- Provide social interaction

It is recommended that all participants visit their General Practitioner prior to starting this program to ensure that there are no medical precautions to commencing exercise

Interpreting Services
Information on how service providers and clients can access interpreting services is available at Translating and Interpreting Service (TIS National).
Sign language interpreting services
Older Australians who are who are deaf, deafblind, or hard of hearing who are seeking to access or are in receipt of Commonwealth funded aged care services can access free sign language interpreting services. Face-to-face sign-language interpreting and Video Remote Interpreting services are available to support clients to engage with:
- Activities of daily living
- My Aged Care
- Regional Assessment Services
- Aged Care Assessment Teams
- In-home aged care service providers
- Residential aged care service providers, and
- Other organisations involved in the provision of Commonwealth funded aged care services.
Sign language services are available in Auslan, American Sign Language, International Sign Language, and Signed English for deaf or consumers who are hard of hearing, and tactile signing and hand over hand for deafblind consumers.
The sign-language interpreting services will support older Australians to better engage and fully participate in their aged care journey. Men’s Shed
The Men’s Shed is a space for men to come together and socialise, enjoy a meal and complete both community and personal projects.
The program provides for health and wellbeing through socialisation and activity including woodcraft, metal working, photography, computer skills training and outings. The Men’s Shed aims to assist men over 55 years of age to remain focused and active.
It provides a meeting place for three days per week.
Other activities include fishing trips, photography and day trips to local points and places of interest.
All men are welcome who are interested in activities, comradeship and a chance to learn.
Is open Tuesday, Wednesday and Thursday.

Community Garden
The Community Garden is available to community members to grow and tend to garden projects.

Domiciliary Midwifery
New mothers are offered support and advice by our community midwives.
Services offered are:
- lactation advice and support
- postnatal monitoring and education
- parent craft education.
Palliative Care
Is offered in the home to support patients and their families with their end of life choices, keeping them as comfortable and pain free as possible during the end stages of life.
Wound care
Individual assessment and treatment as needed.
WD Thomas Day Centre & Planned Activity Group
The WD Thomas Day Centre & Planned Activity Group provide a suitable and varied program to older people and people with a disability in the community.
These programs are designed to maintain and enhance the social, emotional, intellectual and physical wellbeing of the clients and to assist them to maintain their independence.
The service aims to meet the needs of each individual and provide a varied program, promote freedom of choice and provide physically active activities to enhance mobility and confidence.

Welfare Services
Post Acute Care
Day Centre program operates Monday, Wednesday and Friday.
Planned Activity Group operates on Tuesday and Thursday.
Men’s Group outing on the first Monday of the month.
Physiotherapy and Allied Health operate from the Day Centre also.
Strength building, mobility maintenance & enhancement exercise program: Rural Allied Health Team Physiotherapy assessment is undertaken prior to commencement of the programs with individual programming for each participant. Clients are assisted by Vic Fit trained instructor and Allied Health Assistants. Tai Chi sessions run each Tuesday and Thursday.
Men’s Shed: Is open Tuesday, Wednesday and Thursday. All men are welcome who are interested in activities, comradeship and a chance to learn. This program provides for health and wellbeing through socialisation and activity including woodcraft, metal working, photography, computer skills training and outings.
Community Garden: Located in Burgoyne Street near the Men’s Shed.
Community Kitchen: The community kitchen allows people to come together, shop for food, prepare a meal and eat socially together.
Pulmonary Rehab: Caters for people with chronic pulmonary disease and cardiac failure. The program is activated on a needs basis to enhance the lifestyle and fitness level of participants.
Needle & Syringe Program: Assists in reducing the risk of HIV and Hep B and C in our community. Packs are available from the dispensing machine in front of the hospital.
Diabetes: Monitoring and education information including stabilisation.
Continence: Assessment, education and support.
